Road to Recognition – Tools and Tips for Your Journey – - Don’t be daunted by the National Committee for Quality Assurance (NCQA) Physician Practice Connections – Patient Centered Medical Home (PPC-PCMHTM) Standards. While achieving medical home recognition through the NCQA requires measuring and documenting what makes your practice a medical home, the AAFP has produced Road to Recognition, a guide to simplify the process.
